Services Provided
Prime Form Care provides telehealth consultations, clinical evaluations, and treatment plans for men's health conditions including hormone optimization, medical weight loss, and metabolic health. All services are provided by licensed healthcare providers in Virginia. Prime Form Care, LLC utilizes pharmacies believed to meet applicable quality and safety standards; however, variability may still exist.
Eligibility
Our services are available to adults (18+) who are residents of Virginia. By using our services, you represent that you meet these eligibility requirements.
No Guarantee of Treatment
A consultation does not guarantee a prescription or specific treatment. All clinical decisions are made at the sole discretion of the treating provider based on medical history, symptoms, and clinical findings.
Payment & Fees
Prime Form Care is a cash-pay practice and does not accept insurance. Payment is due at the time of service. HSA and FSA cards may be used for qualified medical expenses.
Telehealth Consent
By using our services, you consent to receiving care via telehealth. You understand that telehealth involves electronic communication of medical information and that there are risks including technical difficulties and potential security vulnerabilities.
Limitation of Liability
Prime Form Care and its providers shall not be liable for any indirect, incidental, special, or consequential damages arising from the use of our services, to the fullest extent permitted by law.
Modifications & Contact
We reserve the right to modify these Terms at any time. Continued use of our services constitutes acceptance of any changes.
